Description : It has to be made in the municipality where the death is established. A declaration is generally made by the undertaker's firm. If you wish to make the declaration yourself, you have to report to the registry office
What do you have to take with you? :

the death notification signed by the doctor. And, where appropriate: the deceased person's marriage certificate  or birth certificate, identity card, driving licence, the last will (available from the Population department covering the deceased person's place of residence), burial permit (if the burial took place in another municipality ) or in the event of a cremation, the certificate from the medical examiner.
